TRANSCRIPT REQUEST

Requesting your high school transcripts from SJA is easy!  Simply print a copy of the request form, fill it out and bring it by or drop it in the mail.  Transcript fee is $5.00 per request and must accompany the request form.  When mailing in the form, payment must be in the form of check or money order.  Cash is accepted in person only.  Sorry, no credit cards are accepted for transcript requests.  For further information, please contact Celine Estremera in the Business Office at 414-274-4706.
